Indications
Finasteride is used to treat and control benign prostatic hyperplasia (BPH) by causing the enlarged prostate to shrink.
To enhance the flow of urine
To alleviate the signs and symptoms of BPH.
Pharmacology
Finasteride is a competitive inhibitor of the enzyme 5 reductase that is used to treat benign prostatic hyperplasia (BPH). It has little affinity for androgen receptors and is selective for 5 reductase type 2 enzyme. The conversion of testosterone to dihydrotestosterone (DHT) within the prostate is required for the growth of the prostate gland and consequent BPH. Finasteride is a member of a new class of drugs that target 5 reductase, an intracellular enzyme that converts testosterone to the more potent androgen DHT. The androgen receptor has little affinity for finasteride.
Dosage & Administration
The recommended dosage is one 5 mg tablet daily. Although early improvement may be seen, treatment for at least six months may be necessary to assess whether a beneficial response has been achieved. Thereafter, treatment should be continued.
Interaction
There have been no clinically significant medication interactions discovered. The cytochrome P450 related drug metabolising enzyme system does not appear to be affected by finasteride. Propranolol, Digoxin, Glibenclamide, Warfarin, Theophylline, and antipyrine are some of the compounds that have been tried in humans.
Contraindications
Any component of this drug causes hypersensitivity. Finasteride is also not recommended for women or children under the age of 18.
Side Effects
Finasteride is a drug that is well tolerated. Impotence (3.7 percent), decreased libido (3.3 percent), and decreased ejaculate volume (3.3 percent) have all been documented in clinical research as probably drug-related side events in 1% of individuals treated for 12 months with 5 mg Finasteride daily (2.8 percent ).
Pregnancy & Lactation
Finasteride should not be used by women who are pregnant or may become pregnant. Finasteride is not recommended for women. Finasteride is not known to be excreted in human milk.
Finasteride exposure and the risk of a male fetus: Because of the risk of finasteride absorption and subsequent damage to a male baby, crushed or broken Finasteride Tablets should not be handled by women who are or may become pregnant. Similarly, in subjects using Finasteride 5 mg/day, tiny quantities of finasteride were retrieved from the sperm. It's unclear whether a male fetus may be harmed if his mother is exposed to finasteride-treated sperm.When a patient's sexual partner is or may become pregnant, the patient should either avoid exposing his partner to sperm (for example, by using a condom) or stop taking Finasteride.
Precautions & Warnings
Patients with significant leftover urine volume and/or drastically reduced urinary flow should be closely watched for obstructive uropathy because the favorable response to Finasteride may not be apparent right away.
Prostate cancer: Patients with BPH should have a digital rectal examination and other tests for prostate cancer before starting Finasteride medication and at regular intervals afterward.Finasteride lowers the concentration of prostatic cancer indicators such prostate specific antigen (PSA) in the blood, however this does not rule out the possibility of concurrent prostate cancer in people with BPH treated with Finasteride. Finasteride has yet to be shown to have any clinical benefit in people with prostate cancer.
